We want to make working with PrimeCare as easy as possible - this includes accurate and complete data for each trip. Below are several ways to get your trip on our Schedule.

  • Type of Service - Ambulatory, Wheelchair, Stretcher

  • (if Wheelchair, do we need to bring one?)

  • Patient/Rider First & Last Name

  • Patient/Rider Date Of Birth

  • Patient/Rider approx Weight

  • Will they have an escort riding along (we allow 1 to ride for free)

  • Name of person placing the order

  • Person responsible for payment
