General Description:
Lakeside Behavioral Health System (Lakeside) is a Hospital located at 2911 Brunswick Road, Memphis, TN 38133. They have a large campus with several pick-up and drop-off locations. As this is a behavioral health facility, we need to be mindful, watchful and careful with all patients, both coming and going. We are a secondary provider for them and have had a good working relationship for over 2 years.
Pick-Up / Drop-off Location(s):
Primary: NARC (“Needs Assessment” or “West”), ICAC (“East”), GERO (“Geriatrics”),
Secondary: The Landing, Adolescent, Lobby (front side of NARC), and Cresthaven (building with blue roof).
Off-Campus Pick-Ups: Saint Francis Bartlett, Regional One (The Med), Saint Francis Park, other
Off-Campus Drop-Offs: MMHI (Memphis Mental Health Institute), CAC (Crisis Assessment Center), Discharges home
Types of Service:
We have a contract to provide AMB, WC and STR transportation for Lakeside patients and/or staff, both to and from their Facility.
Transport Procedure(s):
Trips are typically called in by Bill Sanders (901) 581-0273 or by Lakeside Staff. Trip Sheets may not be complete as more information will be needed once we arrive on scene (example: home address for a Discharge). At time of call, Bill and/or Lakeside Staff needs to make sure we get good information on patient status (state of care, flight risk, mental/emotional status, etc.). When getting trip details, ask “What do I need to know about this patient?” to prompt them to disclose pertinent information for safe transport. Make sure we relay this information to our Drivers.

Trips are typically called in when patients are ready to be transported, very few scheduled trips.

When trips are called in, we need AT LEAST the following information: patient name, type of service requested, location for pick-up / drop-off, who called it in, and date/time it was called in.

Unless clearly stated when transportation is requested, ANY patient being taken out of ICAC or NARC should be on a STR for safety and security. We know they can walk and we know they sometimes don’t want to, but this is our agreed upon policy.

STR trips are a two-person team for loading and unloading. For transport, depending on comfort of Driver(s), this can be both people in same van or one Driver can trail the other Driver in seperate van. Lead Driver should keep close eye on patient and should hit flashers and pull over with any issues, Trailing Driver should pull over and immediately rush to assist if they see flashers.

There is typically Lakeside staff with patients returning from Hospitals.

We drop patients off at the address/location given to us. If location refuses or it’s a bad location, contact Dispatch or Lakeside directly for better information. Remember- we are being paid by Lakeside to take the patient to a specific address.

For MMHI trips, you MUST get ORIGINAL stamped paperwork (“Original” stamped in red) or MMHI will not accept patient and you will have to drive all the way back to Lakeside.

When picking up, make sure you get Paperwork and Possessions. Keep Possessions away from patient until you drop them off.

Each building/location has an exterior intercom. Hit button, tell them you are “PrimeCare here for (Patient Name)”.

Double check drop-off location once you get to Facility. Don’t listen to patient as they may be confused and/or medicated. If there is a conflict, contact Dispatch or Lakeside directly.

Lakeside’s “sister” facility is Parkwood Behavioural Health, located at 8135 Goodman Rd, Olive Branch, MS 38654.

We also have a contract with Rhodes College to transport identified students to Lakeside as directed by Rhodes College. In most cases, we will not have the students’ name, as they are protected by Rhodes’ privacy policies, but students will be picked up at the Moore-Moore building on campus, and they will be with the worker who called in the trip. We are obligated to not ask the students their name.
